  • Your full name

  • The name of the police department and officer(s) involved

  • The date and location of the incident

  • A detailed description of what happened

  • Any injuries or damages you suffered

  • Any witnesses to the event

  • Whether you filed an internal complaint with the department

  • Whether you were arrested or cited (if applicable)

  • Your preferred method of contact (phone or email)

  • Your phone number and email address for follow-up

Support and Follow-Up

After you leave your voice message, our staff will review your information and follow up with you within 24 hours.

If you wish to have a live conversation with our team, downloading and using our app is the best option.
The app allows faster communication, voice messaging, and direct updates from staff.
